Since we began, we have been dedicated to working with and assisting aging adults and individuals with disabilities, as well as their families, caregivers, and support teams. Our role is to partner with individuals to oversee and coordinate their support services, enabling them to maintain living in their homes and thriving in their neighborhoods and communities.
Our support services are designed to align with the requirements of the agencies and individuals we engage with. This supports a level of choice and freedom, granting individuals the ability to manage Direct Care Professionals, set schedules, and determine the specific types of services they receive—all within the comfort of their homes and communities.
Our team is made up of dedicated, passionate, and knowledgeable people who are focused on building a strong relationship with our partners to better assist each Participants’ changing needs.

FREEDOM
The ability for a person to plan how they want to live their lives with the support chosen family, friends, or agency assistance.
AUTHORITY
The ability for a person for a person to have control over authorized services, funding and identify who provides his or her supports.
SUPPORT
The collaboration of resources (including natural supports) to assist a person in living a meaningful life in the community.
RESPONSIBILITY
The recognition of a person’s contribution through employment, relationships, spiritual development, and general caring for others, as well as accountability for spending public dollars in ways that are life-enhancing.
